Confidential communication authorization FORM

Many of our patients allow family members such as their spouse, parents, or others to call and
request your medical or billing information. Under the requirements of HIPAA, we are not allowed to
give this information to anyone without the patient's consent.

Please fill out the following form to either accept or deny release of your personal health information to your specified contacts. This form also asks you to specify your preferred means of communication, whether that be through cell phone, home phone, text, voicemail, etc.

WHAT TO BRING TO YOUR EXAM

Please bring the following things to your exam. This will help to expedite the check-in process:
  • Picture ID (Drivers License, State ID Card, or Passport)
  • Vision Insurance card
  • Medical Insurance card
  • Any eyeglasses you use or have recently used
  • Contact lens prescription if you're a current wearer (or a picture of the packaging)
  • List of medications you are currently taking
  • List of any changes to your general health and vision
  • List of questions you have for the Optometrist
  • Welcome Forms (if you downloaded them online and filled them out prior to the appointment)

​WHAT TO EXPECT DURING YOUR EXAM

​Please allot at least an hour for your appointment.
  • In our pre-testing area, an optometric assistant will obtain a peripheral vision screening, eye pressure measurement, and Optos retinal image screening.
  • In the exam room, your optometrist will determine your eyeglass prescription and test your visual function.
  • Your optometrist will then assess the front (anterior) and back (posterior) health of your eyes using the slit lamp or biomicroscope.
  • Once all testing is complete, the doctor will review your findings and discuss your Optos retinal imaging with you. You will then be given recommendations about your eyeglasses and eye health. We will provide you with all applicable eyeglass, contact lens, and medical prescriptions.
  • If you have diabetes or other specific eye health needs (macular degeneration, glaucoma, keratoconus, etc.), you may need additional testing with a dilation exam and/or other highly specialized diagnostic tests. These tests may be performed on the same day or at a follow up visit.
  • You can browse our optical shop where our opticians will help you find the best eyewear for you. They will analyze your insurance benefits to ensure you are able to maximize your coverage.

​INSURANCE

Our staff at Beachside Optometry are experienced in working with insurance providers. We will work directly with your insurer to submit claims regarding your eye care. Most plans often require a co-payment for an eye exam. Please reach out to our staff if you have any questions regarding your insurance coverage and benefits.

We are a provider for the following vision insurance plans:

  • Davis
  • Eyemed
  • Medicare (PPO only)
  • Spectera
  • Superior
  • VSP

We take most PPO medical insurance plans for office visits (including but not limited to):
  • Aetna (PPO only)
  • Anthem/Blue Cross/Blue Shield (PPO only)
  • Cigna (PPO only)
  • Medicare (PPO only)
  • United Healthcare (PPO only)

We are a provider for diabetic dilation exams for:
  • Edinger Medical Group
  • Memorial Care (and glaucoma exams)

Payment options

We accept Cash, Personal Checks, American Express, Master Card, Visa, and Discover.

​We also accept Care Credit and Flex Spending Accounts (FSA). Please contact our office for more details.
